Hours and Appointments
Our office is open Monday through Thursday from 8:00 a.m. to 5:00 p.m., and Fridays from 8:00 a.m. until 1:00 p.m. Medical care is rendered by appointment only. Appointment cancellations must be received 3 business days prior to your scheduled appointment. A cancellation fee may be charged on appointments cancelled without sufficient notice.
If you wish to become a patient, please call our office at 407-331-7007 to discuss your appointment. Once you have made your appointment you may print out your new patient forms from the website.
PLEASE NOTE: Our office does not accept any insurance. We are a fee-for-service practice. If you do have medical insurance we will give you a receipt and statement that you can submit to your insurance company for reimbursement.
Telephone Calls
To better serve our patients and direct all calls to the proper department, we utilize voice mail services. Please listen to the complete menu before making your selection.
Our staff reviews voice mails periodically throughout the day and all calls are returned as promptly as possible.
Please allow a minimum of three (3) business days for prescriptions refills and for messages to be reviewed and calls returned.
If you require immediate medical care , go directly to the nearest emergency room or dial 911.
Patient Intake Forms
New Divine Health patients should fill out the following intake forms before coming in for their first appointment. Many of these forms can be filled out online and then printed for submission to Divine Health.
